"Salud, gratuita y digna" probably means that the people are asking their government for a public health service, free of charge, and that treats them, patients, with dignity. At least, that's one service people pay taxes for. (education, health, etc). "Digna" is the adjective for the noun "Dignidade" - dignity (I don't know the correspondent adjective in english!). In portuguese, it would be almost the same: Saude, gratuita e digna - and in my country that's what it would mean. And people here are overloading hospitals asking for that...
